Replacing a Missing Registration Card, License Plate, or Registration Sticker

So you lost your registration card and are stressing about the hassle you will have to go through at the DMV? Everyone makes mistakes. Registration cards, stickers, and license plates getting misplaced are all extremely common occurrences. Fortunately, the process of obtaining replacement documents is a lot simpler than you would expect.
Whether your registration card, California plates, or stickers are stolen, lost, or not readable in their present condition, the procedure for obtaining a replacement is conveniently similar and simple for each.
It is important to note that you will want to obtain your replacement documents as soon as possible. Driving without proper registration documentation is illegal and can lead to a ticket with a hefty fine.

The Application for Replacement Plates, Stickers, and Documents

It doesn't matter if you need a new registration card, new license plates, or new stickers, you will have to fill out an Application for Replacement Plates, Stickers, and Documents. This document can be obtained at the DMV website or, by coming in our office at 6189 University ave San Diego, CA 92115 instead a trip down to your local DMV office. In order to complete Section A of this document, you will need to provide your vehicle identification number (VIN), license plate number, driver's license number, and the make of your vehicle. In Section B you will need to indicate what kind of document you need replaced. Section C will ask why you need a replacement document. Check the box that most applies to you (choices include Lost/Stolen/Destroyed/Never received, etc.) or check the "other" box and write out your own explanation. Section D only applies if you have requested a license plate replacement. If you have only lost one plate, you will be required to surrender the remaining plate to the DMV. If both plates are missing you will be required to notify law enforcement. Section E Simply sign and date the document.
